New Maryland Medicare Supplement Application
Effective immediately, Maryland’s application has an updated revision. All new business in Maryland is required to be submitted on the new application. Rather than the old revision “NGLMEDAPP10-MD” please use form number “NGLMEDAPP10-MD REV 090518.” The application can be found below, or on the NGL Agent Portal.

Guaranteed Issue – Losing Employer Coverage
A client may qualify for guaranteed issue when losing coverage through an employer group plan. This can happen when the client is the employee or the client was covered under their spouse’s employer group plan, and their coverage is involuntarily discontinued. Please note: the client will only qualify as guaranteed issue if the employer plan paid claims after Medicare paid. If the employer plan does not coordinate benefits with Medicare, then client must apply for a policy including underwriting (if the client’s Part B enrollment date is more than six months ago).
NGL must receive documentation from the employer that includes the coverage start and end dates as well as a statement indicating that the employer coverage paid after Medicare paid in order to process the application as guaranteed issue.
