- Applications may be submitted online through the Producer Portal, producer.highmark.com, or via fax 1-888-663-0258. Please note that you should only submit an application through one channel, and paper applications cannot be uploaded to the Portal.
- If you submit an application online through the Portal, you can confirm the status by selecting “Check Application Status” once logged in. Alternately, if you fax an application, visit medicare.highmark.com and select “Check Application Status” at the top of this web page.
- You have the option to upload a Scope of Appointment when submitting an online application; if you opt not to include one, please ensure you keep this document accessible in your records.
