Exciting Update: Health Assessment Program! You asked and UnitedHealthcare listened, completing a Health Assessment (HA) is easier than ever! They are updating LEAN to include the Health Assessment questionnaires. Coming soon, agents will no longer go to the Revel website to complete a HA. The HA must be completed in LEAN from the My Applications page. No going back and forth between applications, you can complete the Enrollment Application and HA in LEAN! Works in both LEAN Online and LEAN Offline. No additional HA training is required, just complete the attestation each time you start an HA. Application information, including the LEAN confirmation number and Medicare Beneficiary Identifier (MBI), will automatically pull into the HA from the completed LEAN Enrollment Application.
